You have rights surrounding your medical records. At Rogers Behavioral Health, we take the security and privacy of your health information seriously. You can request your records, gain secure access to the patient portal, revoke authorizations to your medical records, and request appropriate amendments.
Call 833-984-3366 Monday through Friday, 8 am to 4:30 pm. CT to make requests about your records.
Finding My Medical Records
Rogers will send you a copy of your medical records at your request. Processing time takes between five to seven business days after your written request has been received. We will notify you if we need additional information to process your request.
Rogers Behavioral Health does not charge for copies of records at this time.

Access Your Records on the HealtheLife App
Register for the HealtheLife app to view your Rogers medical records by calling us at 833-984-3366.

Your medical records are private to you, authorized Rogers staff, and individuals you authorize to view your information. This notice describes how medical information about you may be used and disclosed and how you can get access to this information.
Authorization Forms
You can also request your medical records be sent to an authorized provider or individual. We require written authorization before sending your medical records to any person or organization. Download and complete the Rogers Behavioral Health Authorization form. Instructions are included in the PDF and can be easier to visualize on this sample form.
You can request your records be released via mail, fax, digital delivery, or another method. Indicate how you want your records delivered on the authorization form. Once the form is completed, return it to Rogers via one of the methods at the top of the authorization form.
Revoking Access to Your Rogers Behavioral Health Medical Records
Revoking access you previously authorized must be in writing. Choose appropriate language, complete the form below, and return it to Rogers Behavioral Health to revoke previously authorized access to your records.

Requesting Changes to Your Medical Records
If you believe there is incorrect information in the medical record, please complete a request for amendment. Completed forms should be forwarded to releaseofinformation@rogersbh.org. You will be contacted regarding the changes you requested once we receive your request.
Frequently Asked Questions About Patient Medical Records
You can obtain a Date of Service letter through Rogers Behavioral Health. Authorization should be completed with the Date of Service option (in section four of the authorization form) and sent to Rogers Behavioral Health.
The entire medical record is a complete record of your treatment at Rogers Behavioral Health.
An abstract is a summary of your treatment at Rogers Behavioral Health, which includes the following:
- Psychiatric evaluation
- Discharge summary
- Consultation/history & physical
- Medications
